Park Place provides long-term care for those in need of an increased amount of assistance in their daily lives. We offer compassionate and quality care in a beautiful setting. This includes a family-like community setting, private and semi-private rooms, an outdoor courtyard, sunrooms, a hair salon, along with daily activities and seasonal outings. We provide 24-hour nursing care and a hospice suite if needed. A unique feature of Park Place is our physical connection to Henry County Health Center, a full-service hospital with an Emergency Department. We accept private pay, Medicaid, and long-term care insurance.

After reviewing referral information, we encourage a tour of our facility. During this visit, we highlight the services we offer and describe life at Park Place. This is also an opportunity to address questions or concerns. We encourage visitors to share their needs and goals with our team. Like each resident, each admission is unique. We highly recommend taking this tour to connect with our team so we can better address specific needs and tailor the admission process accordingly. Our goal is to make visitors feel comfortable and empowered in their decision.
The first step in the admission process is to contact our social worker. Our facility will need important documents to manage each specific admission process. Our admission team will carefully review the application, medical and financial information to ensure our facility is a good fit for the individual. On the day of admission, other paperwork will be completed with the resident, family member, or power of attorney, and a Park Place social worker.

Once we know we can meet the individual’s clinical needs and have verified medical and financial information, we will begin gathering admission forms and scheduling the transfer to our facility.
We want Park Place to feel like home. Upon the resident’s arrival, they will be welcomed by our caring team members. Within the first few days, our staff will learn about their interests, preferences, and goals to ensure every aspect of care is tailored to the resident’s needs.
